The Tennessee Department of Health notified the state medical board about the state of Dr. Hill’s patient records, alleging 16 of his medical records were incomplete. The medical board consequently launched an investigation into Dr. Hill’s record keeping skills.

Dr. Hill later admitted to the allegation and voluntarily surrendered his medical license. In addition, he surrendered his federal certificate to prescribe narcotics, according to the report. Dr. Hill was previously disciplined by the medical board first in 1997 for prescribing medically unnecessary controlled substances and then in 2009 for failing to obtain continuing education credits.
