CMS has released the final participant list for its Ambulatory Specialty Model, a mandatory payment model that will tie physicians’ Medicare Part B pay to their performance on low back pain care, starting Jan. 1, 2027.

The low back pain cohort includes 3,319 clinicians, down from about 4,027 on the preliminary list released in February. Participants span 46 states and include orthopedic surgeons, neurosurgeons, pain management physicians, interventional pain physicians, anesthesiologists and physical medicine and rehabilitation physicians.

Under the model, physicians are scored individually on quality, cost, improvement activities and interoperability. The resulting bonus or penalty applies to all of a physician’s Medicare Part B payments, not only those tied to low back pain care.

Here are the 10 practices with the most clinicians in the low back pain cohort, according to the CMS dataset:

  1. OrthoLoneStar (Houston, Tyler, Austin and Dallas, Texas): 31
  2. OrthoNJ (Lakewood-New Brunswick, N.J.): 22
  3. TMH Physician Associates (Houston): 20
  4. The Emory Clinic (Atlanta): 19
  5. Arlington Neurosurgeons (Dallas-Fort Worth): 18
  6. Resurgens Orthopaedics (Atlanta): 17
  7. Lehigh Valley Physician Group (Allentown, Pa.): 17
  8. UT Southwestern Medical Center (Dallas): 16
  9. Physical Medicine Associates (Arlington-Alexandria, Va.): 15
  10. MSK Group (Memphis, Tenn.): 15

At the Becker’s 32nd Annual Meeting: The Business and Operations of ASCs, taking place October 29-31 in Chicago, ASC leaders, surgeons and healthcare executives will explore strategies to drive growth, enhance operational performance, navigate reimbursement challenges and prepare for the future of ambulatory surgery.
