Palomar Health Medical Group, which operates as the outpatient arm of Escondido, Calif.-based Palomar Health, has provided notice of a data security event.
On or around March 11, 2026, PHMG became aware of suspicious activity in its third-party business associate’s hosted environment that contains PHMG data, according to a recent notice from PHMG.
The incident impacted 501 individuals, according to the U.S. Department of Health and Human Services Office for Civil Rights. PHMG reported the event to the office on May 8.
An investigation determined that an unauthorized party gained access to files that included PHMG data. The event did not impact PHMG’s internal systems.
Data that may have been accessed includes names, addresses, dates of birth, Social Security numbers, medical history information, treatment information and medical record numbers.
At this time, PHMG is not aware of any misuse as a result of the incident, the notice said.
The organization suffered a separate security event in May 2024.
PHMG has a network of more than 15 outpatient and urgent care locations, according to its website.
