Philadelphia-based Jefferson Health has been awarded a $1.7 million federal grant from the Patient-Centered Outcomes Research Institute to expand hypertension control efforts across its primary care network, according to an April 29 LinkedIn post.
The initiative, led by Geoffrey Mills, MD, enterprise medical director for primary care quality at Jefferson Health, will implement systems to improve hypertension control across the system’s 145 primary care practices.
Beth Careyva, MD, medical director for value-based care and quality improvement at Jefferson Health’s Lehigh Valley region, is part of the project team supporting the effort.
The program aims to improve cardiovascular outcomes for patients and communities across Jefferson Health’s footprint, according to the post.
