The case management service provides a multidisciplinary care team that works with individuals, their physicians and caregivers to remove complex barriers to care and promote wellness. McKesson has been delivering integrated case management programs successfully for the past six years.
McKesson's services provide a whole-person program identifying and addressing barriers to care, closes gaps in care, enhances care team coordination and helps prevent at-risk individuals from unnecessary hospitalizations and readmissions.
