Here are the six of the worst big cities for physicians to practice by region:

1. Los Angeles, with a high cost of living and intense healthcare market consolidation.
2. Albuquerque, N.M., with a high crime rate and above-average HMO penetration.
3. Detroit, with rampant healthcare consolidation and a bankrupt city.
4. Providence, R.I., an unfriendly tax state with an uncompetitive business climate.
5. Washington, DC, with very high physician density and high cost of living.
6. Miami, in the highly-litigious state of Florida with a high poverty and unemployment rate.
