•    Average patient payment costs increased 11 percent from $2,245 in the fourth quarter of 2013 to $2,491 in the fourth quarter of 2014.
•    The cost of joint replacement procedures rose almost 20 percent from $2,617 in the fourth quarter of 2013 to $3,135 in the fourth quarter of 2014.
•    80 percent of survey respondents said that pre-treatment cost estimates would be "helpful" or "extremely helpful," but only 25 percent reported actually receiving this information.
