Colonoscopy Quality Metrics: Adenoma Detection Rate

Researchers from Indiana University in Indianapolis published a study in the journal Gastrointestinal Endoscopy focused on determining the optimal measurement method for adenoma detection rate in colonoscopy.

The researchers set out to define benchmarks for an "adenomas per screening" colonoscopy quality metric and explore adenoma detection rate in screening, surveillance and diagnostic indications.

The researchers studied a total of 21,766 colonoscopies. Of these colonoscopies:
•    34.2 percent were screening
•    38.3 percent were surveillance
•    27.5 percent were diagnostic

The researchers found that ADRs were correlated with APCs.

•    In men, a 25 percent ADR corresponded to an APC of 0.46
•    In women, a 15 percent ADR corresponded to an APC of 0.20
•    For men, a screening ADR of 25 percent corresponded to a surveillance ADR of 31.9 percent
•    For women, a screening ADR of 15 percent corresponded to a surveillance ADR of 24.3 percent

The researchers concluded by suggesting minimum APC detection benchmarks of 0.50 for men and 0.20 for women.
