The Institute for Safe Medication Practices offers a free sample failure mode and effects analysis for anticoagulant therapy.

The tool is designed to help healthcare providers determine the risks associated with anticoagulant therapy and their ability to eliminate them. The FMEA enables providers to score anticoagulant therapy-related processes for severity, probability and likelihood of detection and assign each process a risk priority number.
