Patient Safety Tool: Medication Safety Checklist

The National Council on Patient Information and Education offers a free, downloadable checklist to help patients adhere to medication plans and avoid medication errors.

The "Make Notes & Take Notes" form includes a checklist of basic information patients should ask their physician about, including symptoms and current prescriptions. The checklist also includes room for answers to questions about medications, including medication schedules, potential side effects and storage requirements.
