The Health Research & Educational Trust, an affiliate of the American Hospital Association and supporter of Partnership for Patients, offers a free guide on reducing harm from high-alert medications.

The guide, "Implementation Guide to Reducing Harm From High-Alert Medications," is designed to help healthcare providers implement changes that will reduce adverse drug events. The tool includes recommendations for standardizing care processes, avoiding errors during care transitions and using decision support, among other topics.
