Patient Safety Tool: BOOST Care Transition Implementation Guide

The Society of Hospital Medicine offers a free implementation guide for Project BOOST — Better Outcomes for Older adults through Safe Transitions — a national initiative to improve care for patients as they transition from hospital to home.

Project BOOST aims to reduce 30-day readmission rates for general medicine patients, reduce length of stay, improve patient satisfaction and smooth the transition from inpatient to outpatient care. The BOOST toolkit, "Implementation Guide to Improve Care Transitions," includes quality improvement tools, steps for developing and evaluating interventions and various other resources.
