A study from BMC Medical Informatics and Decision Making surveyed Boston physicians about managing their EHR systems and found the problem list to be the most controversial element, according to EHR Intelligence.
Largely, practitioners differ in their approaches to maintaining the problem list, which is a compilation of clinically relevant physical and diagnostic concerns, procedures and cultural issues that may affect the care of a patient.
These lists can easily become filled with too many irrelevant facts or incomplete and missing critical information for care. Current standards focus mostly on coding rather than daily physicians use.
Creating policies to uniform the procedures for maintaining the problem list may help avoid incomplete or overcrowded health records.
