Cedars-Sinai and CareMore Health to offer coordinated care for Medicare patients: 5 things to know

Cedars-Sinai, a Los Angeles healthcare provider, and CareMore Health Plan, a Medicare Advantage plan, entered an agreement to provide better care for Medicare patients.

Here are five things to know:

1. Cedars-Sinai physicians and CareMore healthcare teams will work together to keep patients out of the hospital by coordinating care.

2. CareMore will open a comprehensive care center close to Cedars-Sinai.

3. More than 60 primary care doctors and about 130 specialists from Cedars-Sinai will treat Medicare patients in the program.

4. This care center is open to patients with chronic diseases, hypertension and other conditions, as well as to patients who seek nutritional counseling, strength training and other services.

5. CareMore will also make house calls for those patients who are unable to travel to the care center.

"The strategic alliance will expand access to comprehensive care of the highest quality to Medicare beneficiaries in the Los Angeles area," said Tom Gordon, CEO of Cedars-Sinai Medical Network.
