Here are the 11 top-rated insurers for the lowest number of denials, according to the Medscape Insurer Ratings Report 2014.

Physicians were asked to rate frequency of denials on a scale of one (which means frequent denials) to five (which means infrequent denials).

 

•    Medicare — 3.2 (on a scale of 1 to 5)
•    Blue plans — 3.1
•    Aetna — 3
•    Cigna — 3
•    Medical Mutual of Ohio — 3
•    Kaiser Foundation Health — 2.8
•    Harvard Pilgrim Health Care — 2.8
•    United Healthcare — 2.8
•    HealthNet — 2.8
•    Oxford Health Plans — 2.8
•    Humana — 2.8
