Patient Safety Tool: Safe Site Recommendations for Interventional Radiology

The Minnesota Hospital Association provides a free, downloadable "safe site recommendations for interventional radiology" document.

 

The three-page document shows how many wrong procedure events take place during interventional radiology procedures and provides recommendations pertaining to site marking for procedures done under radiologic image-guidance when the procedure site is not predetermined and site marking for procedures done under radiologic image-guidance when the procedure site is predetermined

 

The document also gives an overview of time-out recommendations.

 

The material in the document can be adapted for use as a checklist or educational sheet on the importance of safe site practices during interventional radiology procedures.
