16 Organizational Changes to Inform AAAHC About

Accredited organizations must notify the Accreditation Association for Ambulatory Health Care in writing, within 15 calendar days, of significant organizational, operational or financial changes.

These changes include but are not limited to:

1. Mergers
2. Change in majority interest
3. Consolidation
4. Bankruptcy or other significant change in the financial viability of the organization
5. Name change
6. Relocation
7. Additional services or locations
8. Major renovations
9. Any interruption in service that exceeds 30 calendar days
10. Death or incapacitation of the physician or dentist in solo physician or dental organizations
11. Changes in state license or federal certification or qualifying status
12. Any governmental investigation, including local, state or federal authorities, involving directly or indirectly the organization or any of its members
13. Adverse publicity or adverse media coverage
14. Criminal indictment, guilty plea or verdict involving, directly or indirectly, the organization or its members
15. Significant changes in managed care enrollment
16. Significant changes in a managed care organization or staff membership
