• Outpatient clinic — 74 percent
• Academic (non-hospital), research, military or government setting — 67 percent
• Office-based solo practice — 64 percent
• Office-based multispecialty group practice — 59 percent
• Hospital — 55 percent
• Office-based single-specialty group practice — 54 percent
